All iPads are property of Sumner County Schools and will be monitored. School internet filters are in place and will work at home and school. iPads will be returned to school if the student transfers/graduates, or when the school year ends.

Fill out and print the Preparticipation Physical History and Evaluation Form

Follow instructions to create and maintain a Final Forms Account for each athlete

**Every athlete must complete these steps before tryouts and participation in their sport. (Physicals are required for Summer activities as well as during the school year)

Physicals are good for 1 school year and not 1 calendar year. Everyone must obtain a new physical each year after April 15th.
